Forms and polls let your community collect information, feedback, and even payments from residents. This article walks through how to fill out and submit a form or poll, what each question type looks like, and how to review your past submissions.
Any community member can fill out a form or poll, provided the form is active and they are part of the form's target audience. Some options may be unavailable or appear differently depending on your level of account access.

Log in to Communitrak at https://app.communitrak.com.
Click Forms & Polls in the main navigation menu.
Find the form or poll you want to complete and click its card, or click the Complete Form (or Complete Poll) button.
The form page displays the type label (Form or Poll) and the form title at the top. Read any notices that appear:
If you have already submitted this form, a blue banner shows the date of your last submission and a View Response button.
For polls, a confidentiality notice explains whether your responses will be anonymous or linked to your profile.
If the form includes an impact area selector at the top, choose the affected property, amenity, or other area. This field is required.
Answer each numbered question. Required fields are marked with a red asterisk (*). See the question type reference below for details on each input.
Click Submit Responses at the bottom of the page.
A success dialog confirms your submission. From there, click View My Submissions to see your response or Done to return to the forms list.

Forms can contain a variety of question types. Here is what to expect for each:
Short Answer -- a single-line text input for brief responses.
Paragraph -- a multi-line text area for longer responses.
Multiple Choice -- radio buttons where you pick one option from a list.
Checkbox -- a yes/no toggle. Some checkboxes include a label next to them (for example, agreeing to a statement).
Dropdown -- select one option from a dropdown list.
Multi-Select -- checkboxes where you can pick more than one option. If the form builder set minimum or maximum selection limits, a hint is shown below the question.
Number -- a numeric input. Minimum and maximum values may be enforced, and a hint is displayed when they are.
Date -- a calendar date picker.
File Upload -- drag and drop a file or click to browse your device.
Signature -- a digital signature pad. Draw your signature with your mouse or finger, then use the clear button to redo if needed.
Property -- a searchable dropdown of community properties.
Payment -- displays payment line items with amounts. Select the items you want to pay for. A running total is shown. You must check the authorization checkbox ("I authorize [amount] to be charged to my [card] ending in [last four digits] when I submit this form.") before submitting. Your default payment method on file is used.
Text Block -- read-only instructional text placed by the form creator. No response needed. Text blocks are not numbered.
Click Forms & Polls in the main navigation menu.
Click My Submissions in the top-right corner of the page.
The My Submissions page shows cards for every form and poll you have submitted. Each card displays:
A type badge (Form in blue, Poll in purple)
The form title
The date you submitted
An "Anonymous submission" badge if the form was confidential
Click any card to open a detail dialog showing all of your responses exactly as submitted. Responses are read-only and include file previews, signature images, payment status (Pending or Refunded), and property addresses where applicable.

If you haven't submitted any forms yet, you'll see a "No Form Submissions Yet" message with a View Available Forms button to get started.
The Forms & Polls module only appears in the main navigation menu if it is enabled for your community.
A form may have completion limits. If you've reached the limit, the form page will display a message explaining why it's unavailable.
Closed or draft forms cannot be submitted. You'll see a notice if you try to access one.
For polls marked as anonymous, your responses are not linked to your profile. Community Leaders reviewing results will not see your name.
